Clinical Pharmacist

Vacancy Introduction

Woodsley and Holt Park PCN is looking for an enthusiastic full-time pharmacist (5 days a

week), to join the well established team of 8 pharmacists and 4 pharmacy technicians. 

This role will give you the opportunity to develop your clinical skills in a patient-facing environment.

The PCN is made up of ten GP practices covering a diverse population in Leeds. You will work

closely with the GP practices and have the benefit of support from General Practice Staff, the

existing pharmacy team and links into the wider MDT employed by the PCN.

The PCN aims to provide the best patient care possible. You will work in a patient-facing role, supporting patients to get the most benefit from their medicines through Structured Medication Reviews. 

You will also provide support to the wider multi-disciplinary team with: 

• Medication and prescription queries, 

• Acute requests for medication, 

• Medicines reconciliation following transfer of care, 

• QOF/IIF/local incentives, 

• Prescribing processes, 

• Medicines policies and procedures, 

• Audit and medicines safety. 

You will be a registered pharmacist and have experience of working as a pharmacist in a community, primary care or hospital setting. You will be committed to providing outstanding patient care. You will have exceptional communication skills. You will be good team player, while being capable of independent and autonomous work. As part of this role you will complete the CPPE Primary Care Pharmacy Education Pathway and undertake Independent Prescribing training, if not already completed.
