Why the Recovery Hubs?

For many people, home first is the ideal outcome when leaving hospital. While increased care at home is often preferred, some patients need a short period of support before they can return safely.

That’s where our Recovery Hubs come in.

We provide short‑term, rehabilitative care that supports:

  • Early discharge from hospital
  • Reduced hospital admissions
  • Safe, supported transition back home

Our teams deliver complex, person‑centred rehabilitation for older adults across Leeds, working alongside a dynamic and skilled multidisciplinary team. Every care plan is built around the individual — their goals, their strengths, and their journey home.

Where You’ll Work

Our Wharfedale Recovery Hub and Community Care Beds Recovery Hub operate 24/7, providing continuous, high‑quality care through a rotating shift pattern.

Community nursing within the Recovery Hubs is ideal for someone who can work confidently both independently and as part of a wider multi‑disciplinary team. You will be resilient, motivated, and adaptable, thriving in a fast‑paced and constantly evolving environment. Above all, you will bring a genuine passion for delivering the highest standard of care to every individual.

You will have excellent communication skills and consistently demonstrate a high level of clinical competence when carrying out nursing procedures. You will also play an active role in ensuring full compliance with our clinical governance framework, supporting and motivating colleagues, and championing service user safety, dignity, and wellbeing. Your commitment to continuous improvement will help drive the ongoing development of our service.
