Vacancy Introduction
An exciting opportunity has arisen to join the Leeds Community Diabetes Service as a Band 4 Facilitator. We are looking for a motivated, organised, and compassionate individual to support the delivery of high-quality diabetes care across the community. This is a permanent and full-time position.
Working as part of a multidisciplinary team, you will play a key role in enabling patients to better manage their diabetes through education, engagement, and personalised support. You will contribute to the coordination and facilitation of structured education programmes, clinics, and community-based initiatives aimed at improving health outcomes and reducing health inequalities across Leeds.
No two days will be the same in this varied and rewarding role. You will be travelling across the city to support delivery in a range of community settings, building relationships with patients and colleagues while helping to ensure services are accessible and responsive to local need.

In this role, you’ll support people across Leeds to better understand and manage their diabetes in their day-to-day lives. You’ll work directly with patients in community settings, helping to deliver education sessions, supporting clinics, and promoting self-management in a clear and practical way.
A typical day may involve facilitating an education session in one location and travelling to another site to run a clinic. As this is a citywide role, you will need to be able to travel independently. You’ll build positive relationships with patients, offering reassurance and tailored information to meet individual needs.
You’ll work both independently and as part of a multidisciplinary team, organising your workload while working within your scope of practice. You will use your clinical judgement appropriately within agreed pathways and seek guidance when needed. Training and ongoing development will be provided to support you in the role.
You’ll help ensure clinics run smoothly, maintaining accurate records and managing resources effectively. You will also be responsible for recognising and raising any concerns or queries with the relevant clinician in a timely and appropriate way.
We are looking for someone who is organised, adaptable, and approachable, with strong communication skills and a passion for supporting people with long-term conditions. You’ll need to be flexible and confident working across a range of community environments in a role where no two days are the same.
