Are you a band 5 or current band 6 nurse who has an interest in diabetes and would like to expand your knowledge and skills in diabetes care and management? If so the Leeds Community diabetes service in Leeds has a role for you in the team.
We have an exciting opportunity for a diabetes specialist nurse to join our multi disciplinary team, made up of nurses, dietitians, GPsi's, diabetes facilitators and pharmacists. If successful you would be working across Leeds to provide face to face and telephone clinics, carrying out home visits and providing education groups for patients with type 2 diabetes.
This role would suit someone who is enthusiastic and committed to providing excellent diabetes care, and who wants to further develop their knowledge and skills in this speciality. We, as a team, are committed to staff wellbeing and development and the successful candidate will be supported to completed NMP if not already done, so as this is an essential requirement of the role.
The role requires excellent communication skills as you will undertake complex assessments of patients with diabetes, implement treatment plans and then review and assess, all while promoting self management for the person with diabetes.
Car owner/driver or suitable alternative transport to enable you to undertake the job is essential (it is unlikely that public transport will meet this requirement).
