Dietetic Associate Practitioner

Vacancy Introduction

Leeds Community Healthcare (LCH) is pleased to be expanding our Home Enteral Feeding (HEF) service and is recruiting two full-time (2 x 1.0 WTE / 37.5 hrs) permanent Band 4 Dietetic Assistant Practitioners to support our specialist Learning Disabilities HEF team. We welcome applications from candidates seeking full-time hours, or part-time/job-share arrangements.

Based within our friendly multidisciplinary team at Parkside Community Health Centre, this role will support the delivery of person-centred nutritional care to adults with learning disabilities who require enteral nutrition. You will work closely with Specialist Dietitians, contributing to assessments, reviews, education and practical care in community settings including family homes, supported living and care homes. Hybrid working is supported where appropriate, and you will receive structured supervision and opportunities for development within this specialist area of practice.

Support delivery of high-quality, person-centred nutritional care within the HEF team for adults, predominantly with learning disabilities, requiring enteral feeding, you will:

-Manage a defined caseload of clinically stable, lower-complexity patients in line with agreed protocols/care plans, under the supervision of a registered Dietitian
 -Monitor nutritional intake/hydration/tolerance/weight, escalating changes in clinical complexity appropriately
 -Provide first-line advice to patients/families/carers, and troubleshoot common enteral feeding and equipment issues
 -Identify feeding concerns and make recommendations regarding feeds or regimens for review and approval by supervising Dietitian
 -Liaise with learning disability teams/care providers/families and contracted feed companies to support coordinated care
 -Support education and training for patients, carers & care staff, adapting communication to individual needs
 -Maintain accurate clinical documentation & contribute to audit, service evaluation
