Discharge Case Manager

Vacancy Introduction

Band 5 Discharge Coordinator – Short Term Care Beds Service

Looking for a new challenge?

An exciting opportunity has arisen for a Band 5 Discharge Coordinator to join the Short Term Care Beds Service on a permanent part-time basis (15 hours per week).

The Short Term Care Beds Service provides short-term rehabilitation and recovery support for individuals who no longer require acute hospital care but are not yet able to return home independently. Working within a Home First approach, the service focuses on supporting timely, safe, and person-centred discharges, enabling people to recover in their own homes whenever possible.

As a Discharge Coordinator, you will play a key role in coordinating discharge plans, working closely with patients, families, community services, social care teams, and the multidisciplinary team to ensure seamless transitions from inpatient care. You will support effective discharge planning from admission, helping to identify and address barriers to discharge while promoting patient choice, independence, and recovery.

You will work alongside Nurses, Therapists, Healthcare Support Workers, Social Workers, and community partners to ensure patients receive the right support at the right time. Strong communication, organisational skills, and a proactive approach are essential.

This is an excellent opportunity for an enthusiastic and motivated Band 5 Discharge Coordinator to gain experience within a community rehabilitation and intermediate care setting. Working within a Home First approach, you will support patients to return home safely and as independently as possible following a period of illness, rehabilitation, or hospital admission.

You will work closely with patients, families, carers, and partner agencies to coordinate safe, timely, and person-centred discharges. The role involves identifying and addressing barriers to discharge, facilitating referrals, coordinating support services, and ensuring appropriate care arrangements are in place to promote successful transitions from inpatient care.

As part of a multidisciplinary team, you will collaborate with Nurses, Physiotherapists, Occupational Therapists, Healthcare Support Workers, Social Care teams, and community services to achieve the best outcomes for patients.

Key responsibilities include coordinating discharge plans, supporting patient flow, liaising with internal and external stakeholders, monitoring discharge pathways, and ensuring patients receive the right support at the right time.

This role offers valuable experience in discharge planning, rehabilitation services, integrated working, and care coordination. You will receive regular support and development opportunities while helping patients achieve positive outcomes and maximise their independence within their own homes and communities.
