We are seeking an enthusiastic, experienced, and highly motivated District Nurse to join our Neighbourhood Teams.
You must have undertaken and passed the Community Specialist Practitioner – District Nurse Course with V300 prescribing qualification. This is an exciting opportunity for an individual with a passion for delivering outstanding patient-centred care within patients' homes and community settings.
As a Band 7 District Nurse, you will provide expert clinical care and leadership, working collaboratively with multidisciplinary teams to support patients with complex clinical needs, promote independence, and prevent unnecessary hospital admissions.
We are in the incredibly privileged position of caring for people in their own homes – generally, the place they prefer to be treated. Forging new relationships within Primary Care Networks and learning from our recent experiences has placed us in a valuable position to rapidly respond when needed.
The successful candidate will play a pivotal role in delivering high-quality, responsive community nursing services while supporting service development and mentoring junior staff.
We are recruiting Qualified District Nurses to provide clinical leadership and oversee the delivery of high quality care within Neighbourhood Teams. You will manage a clinical caseload and lead a cluster of Staff Nurses and Clinical Assistants, to provide excellent care for our patient population.
Our Neighbourhood Teams offer seamless proactive support and care in the community. The teams are made up of multi-professionals working closely with our partners.
Our service runs from 8am to 8pm every day of the year. We are looking for a nurse who is dynamic, innovative, passionate and committed with experience of case management and multidisciplinary work.
We are looking for leaders who possess excellent communication skills and the ability to motivate others. We want individuals who are resilient and able to support others through change to maintain the delivery of high quality, compassionate, proactive and safe care.
Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
We currently have 2 secondment positions available for 9 months each initially with the opportunity for them to be extended.
