Vacancy Introduction
Do you want a role where you can focus on your physiotherapy skills and rehabilitation? Would you like training/development opportunities to further your knowledge and skills in falls prevention? Do you want to work in a specialist service providing evidence-based care to patients at high risk of falls? If so, then this job could be for you.
An exciting opportunity has arisen for a Band 6 Physiotherapist to work in the specialist citywide Leeds Community Falls Service. In line with ongoing developments across the citywide falls pathway, we continue to develop new ways of working to implement an innovative service model, to support older people to maintain their independence, reduce their risk of falling, and provide care closer to home.
As a falls specialist you will work as part of a multidisciplinary team (MDT) in the community setting alongside an Advanced Practitioner, Specialist Physiotherapists, Therapy Assistant Practitioners, and Pharmacist. There will be an opportunity to develop specialist skills in vestibular assessment/rehabilitation. The team also works closely with Consultant Geriatricians, with twice monthly virtual falls MDT meetings to support management of complex high risk falls patients in the community.
The Community Falls Service operates Monday to Friday 8am to 4pm. This role is a new role supporting the existing specialist service provision and supporting new work to provide falls training to care homes across Leeds.
Main duties of the role:
• Providing specialist comprehensive falls assessments and interventions to complex high risk falls patients in the community
• Providing specialist physiotherapy assessments and evidence-based interventions to this client group with highly complex presentations and multi-pathologies
• Providing vestibular assessment and rehabilitation as indicated
• Identifying and discussing patients with the service Advanced Practitioner who may require discussion at the virtual falls MDT meetings
• Providing initial assessment clinics in the community for people referred to the Active Leeds Strength and Balance programme
• Acting as a source of specialist advice, and providing education and training on falls assessment, prevention and management to other healthcare professionals and care homes
• Providing a triage role through the integrated falls referral pathway
You will need to demonstrate excellent communication skills and have experience of MDT working with older people. Experience of community working and falls assessment/management is also essential for this role. Due to the nature of the role and citywide service provision, a car driver is essential.
