Leeds Community Healthcare Trust are looking for Band 5 Physiotherapist to get involved with our Core Therapy or Active Recovery Neighbourhood teams. Our interventions are exciting and dynamic, supporting patient flow through rapid response/admission prevention work and facilitation of hospital discharges with a Home First approach.
As a Trust we support your development across all areas of practice. We commonly see patients for Falls reduction and prevention work, rehabilitation and case management of long-term conditions including Neuro, MSK, Orthopaedics, Respiratory, Rheumatology, Frailty and Palliative and end of life care. Community working allows for a diverse range of cases with #No2daysthesame.
You will have an opportunity to make a positive difference to your patients’ lives and develop skills. You will work closely with the wider multidisciplinary team to enhance your knowledge, and will be supported by clinical supervision, peer support and regular continuing professional development. You will be supported by senior therapists, but you will be responsible for your own caseload delivering holistic rehabilitation or delegating to others. You will develop holistic skills working with the wider multidisciplinary team and will receive regular clinical supervision allowing you to hone your skills with view to progression within the Trust.
The service functions over 7 days from 8am and 6pm, and you will be required to lone work. We promote a positive work/life balance and utilise flexible working to accommodate your needs. We have posts in the North, West and South hub area with an understanding that this is a city–wide service and there may be a requirement for cross cover working, and consideration will be given to any caring responsibilities. Your area of work will be dependent on preference and service needs.
Access to a car that you can use for work purposes is essential.
