A fantastic opportunity has arisen for enthusiastic and dynamic Staff Nurses to join our Integrated Neighbourhood Teams.

Our service is at the heart of the NHS long term plan, providing more care in community settings, such as patients homes, whilst enabling early discharge from hospital and reducing hospital admissions. When you join us, we will work with you to develop your own personal development plan. We won't just stop there though, we are proud of our career progression offer that will allow you to develop your clinical, leadership and preceptorship skills, supporting you to undertake training and offering District Nursing and Community Matron apprenticeships.

The service runs 7am to 10pm every day with various shift patterns. We understand that people have busy lives, and we offer flexible work opportunities.

You will work alongside District Nurses, Community Matrons, Occupational and Physiotherapists, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners and Self Management Facilitators in one MDT to provide truly holistic care for our patients. You will work closely with colleagues in our Citywide Service such as the Falls, Continence, Urology and Colorectal Care and Palliative and End of Life Care teams and GP services to co-ordinate the best care for patients. You will have great communication skills and maintain a high standard of clinical work when carrying out nursing procedures. You will provide person centred care, helping patients to reach their goals and promoting self management. You will be resilient, motivated, and adaptable to working in an ever-changing environment and of course, a passion for providing the best possible care. You will ensure compliance with our clinical governance framework, you will motivate colleagues, champion service user safety and dignity.
