Senior Clinical Pharmacist

An exciting opportunity has arisen for an experienced pharmacist to lead the pharmacy team, in an innovative and forward thinking Primary Care Network (PCN).

It has been recognised there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice. 

Clinical pharmacists play an important role, complementing other members of the multi-disciplinary team. You will work to support patients, the PCN, and General Practice staff in providing patient-centred care, while leading and developing the pharmacy team.
Woodsley and Holt Park PCN are committed to providing a pharmacy service to pro-actively help patients to stay safe and well and out of hospital.  We are looking for a dynamic and skilled pharmacist who would be interested in developing their career in a leadership role. You will be joining a large, well-established team of pharmacists and pharmacy technicians and be responsible for the leadership and management of the surgery-based team to ensure delivery of exceptional care for our patients.

The PCN is made up of ten practices covering a diverse population in Leeds. You will provide a clinical service to two of these practices in addition to the management role.
You will have the benefit of support from General Practice staff, the existing pharmacy team and links into the wider MDT employed by the PCN.

The successful candidate will 
  • Provide leadership and day to day management of the pharmacy staff working directly with the practices of the PCN. 
  • Be responsible for managing medicine optimisation services and lead on quality improvement while supporting local and national priorities such as the Investment and Impact Fund, Quality Outcomes Framework, General Practice Outcomes Programme.
  • Work within their clinical competency as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation and to support patients in getting the best from their medicines. This will include, but not be limited to, conducting Structured Medication Reviews, completing medication reconciliation activities on transfer of care, supporting medication optimisation after long term condition reviews, advising on minor ailments, actioning acute prescription requests and undertaking clinical audit activities.
  • Have an Independent Prescriber qualification and ideally have completed the CPPE Primary Care Pharmacy Education Pathway. 
You will be a registered pharmacist who has had significant post-registration experience in any pharmacy sector, ideally some of that will be in a PCN environment.

You will be enthusiastic about clinical pharmacy and leading service development to grow your own capabilities and those of the clinical pharmacy team. 
You will be committed to providing outstanding patient care.
You will have exceptional communication skills.

Woodsley and Holt Park PCN consists of two neighbouring networks with a population of over 115,000 patients. 

Woodsley PCN consists of Hyde Park Surgery, Burley Park Medical Practice, Craven Road Medical Practice, Vesper Road Surgery, Burton Croft Surgery, Kirkstall Lane Medical Centre, Laurel Bank Surgery and Abbey Grange Medical Practice. 

Holt Park PCN consists of Ireland Wood & Horsforth Medical Practice and High Field Surgery.

The PCN works in partnership with Leeds Community Healthcare to employ PCN staff.
