Leeds Community Healthcare NHS Trust is pleased to offer an exciting opportunity for a dynamic and motivated Advanced Clinical Practitioner (ACP) to join our citywide Home Ward Frailty Service. We welcome applications from qualified ACPs.

About the Role:

You will be based within one of three localities across the city, covering a defined area within East, South or West/North Leeds.
