Specialist Pharmacist – Diabetes PCN

An exciting new opportunity has arisen to work as a specialist diabetes pharmacist, in an innovative and forward thinking PCN. 

Woodsley and Holt Park PCN are committed to developing a multidisciplinary Diabetes Support Team (DiaST) to support with the care of people living with diabetes and those who have been newly diagnosed. We are looking for a pharmacist who has specialist experience in diabetes to join a team of dieticians, health and well-being coaches and a podiatrist, to improve the care of people mainly (but not exclusively) with type 2 diabetes. This exciting new role will give you the opportunity to develop your specialist clinical skills in a patient-facing, primary care environment, working with a wide variety of healthcare professionals. You will be able to access support from senior clinical pharmacists specialising in diabetes across the city of Leeds and the consultant pharmacist for diabetes based at the West Yorkshire Integrated Care Board.

The PCN is made up of ten practices covering a diverse population in Leeds. You will provide your own clinics across the geographical area within GP surgeries and hub sites.
You will have the benefit of support from General Practice staff, the existing pharmacy team and links into the wider MDT employed by the PCN.

  • To work as a clinical diabetes specialist as part of the multidisciplinary specialist diabetes team.
  • To work as an autonomous practitioner, co-ordinating your own caseload to deliver an expert clinical service within diabetes
  • To provide specialist clinical advice to people living with diabetes, their carers,  and other clinical staff to provide safe, effective and evidence based diabetes care.
  • To ensure that people with diabetes are fully informed and supported when making decisions about treatment.
You will be a registered pharmacist and have experience of working with people with diabetes.
You will be committed to providing outstanding patient care.
You will have exceptional communication skills.
You will be good team player while being capable of independent and autonomous work.
As part of this role you will complete the CPPE Primary Care Pharmacy Education Pathway and undertake Independent Prescribing training, if not already completed.

Woodsley PCN consists of Hyde Park Surgery, Burley Park Medical Practice, Craven Road Medical Practice, Vesper Road Surgery, Burton Croft Surgery, Kirkstall Lane Medical Centre, Laurel Bank Surgery and Abbey Grange Medical Practice.
Holt Park PCN consists of Ireland Wood & Horsforth Medical Practice and High Field Surgery.

The PCN works in partnership with Leeds Community Healthcare to employ PCN staff.
