Therapy Assistant Practitioner

Vacancy Introduction

Would you like a role working in an adult community setting offering variety, #no2daysthesame, working with a range of professionals in dynamic, innovative, and supportive teams?

We have an exciting opportunity to work in our Neighbourhood teams in Leeds, alongside physiotherapists and occupational therapists as a Band 4 Therapy Assistant Practitioner. We are looking for someone with previous experience of health and social care, with excellent communication and organisational skills, looking to further enhance their skills in person centred rehabilitation.

You will have the opportunity to organise and manage your own caseload, so we are looking for someone who can work independently within scope, using their own initiative. We facilitate our service users to optimally manage their health conditions, promote independence with daily activities, and enable early discharge from hospital or community-based beds, working towards goals to improve health and wellbeing outcomes.

The role will be based within the West Neighbourhood Teams, this includes Armley, Pudsey, Woodsley, Yeadon & Holt Park.

Key responsibilities are delivering holistic rehabilitation, which includes provision of equipment, teaching and progressing exercise programmes and signposting to community resources to encourage social engagement. You will also be supporting progression of ability to self-manage activities of daily living, including meal preparation, personal care and medications. You will have opportunities to assist with student placements, help develop others, contribute to service improvement projects, and enhance your own skills, including an option to work towards applying for a degree apprenticeship.

The service is provided over core hours of 8am – 6pm across 7 days. Weekend and unsocial hours attract enhanced pay. We support a positive work life balance so when aligning you to a team we aim to consider your personal preference and consider any caring responsibilities that you may have. At times you may be required to support other teams to support workflow and meet service demands.
