The Neighbourhood Night Service operates from 20:45 – 07:15 providing care across Leeds to patients requiring nursing intervention in their own home.
A key aim of the service is to prevent unnecessary hospital admissions and to ensure that palliative patients remain in their own homes where this is their preference. The service is part of a multi-disciplinary team and ensures that there is 24/7 nursing care for patients in the community.
We will support you with a tailored induction programme and will encourage and facilitate on-going development, including the opportunity to develop and learn skills.

This is an exciting and varied role that incorporates many different aspects of patient care delivered to a wide range of patients.
You will support our palliative patients, providing one to one care in their own homes in their last weeks of life. When working with a palliative patient you will be based in the patient home overnight.
You will support the Registered Nurses, responding to intervention call outs overnight or to work alongside a colleague and visit patients overnight for a planned intervention.
You will also support the Virtual Ward (Frailty) providing coordinated rapid care to people in their home who are experiencing an acute medical episode.
It would be expected that you are available to work on a rota over 7 nights per week, 365 days per year. Rotas would be issued with a minimum of 4 weeks’ notice.
Your hours of work will be 21.45-07.15
The contract is for a minimum of 18 hours (2 nights) up to a maximum of 36 hours (4 nights). Hours can be discussed prior to application or at interview as required.
