An exciting opportunity has arisen for a highly motivated and enthusiastic Band 6 Physiotherapist to work in the Leeds Community Falls Service. The citywide service runs Monday to Friday 8am-4pm. In line with ongoing developments in the Trust we continue to develop new ways of working and implement innovative pathways, to support older people to maintain their independence and reduce their risk of falling, providing care closer to home.

Main duties of the role:
As a falls specialist you will work as part of a team in the community setting, alongside other Physiotherapists, an Advanced Clinical Practitioner, Pharmacists, and Therapy Assistant Practitioners. There will be an opportunity to develop specialist skills in vestibular assessment and rehabilitation. The team also work closely with Consultant Geriatricians, with twice monthly virtual falls multidisciplinary (MDT) meetings to support management of complex high risk falls patients in the community. You will be responsible for:
- Providing comprehensive multi-factorial specialist falls risk assessments and interventions to complex high risk falls patients in the community
- Providing specialist physiotherapy assessments, and evidence-based and goal-orientated interventions to this client group with highly complex presentations and multi-pathologies
- Acting as a source of specialist advice, support and education regarding falls assessment, prevention and management to other community healthcare professionals
- Providing a triage role through the integrated falls referral pathway
You will need to demonstrate excellent communication skills and have experience of MDT working with older people. Experience of community working, and falls assessment and management is also essential for this role.
Due to the nature of the role, a car driver is essential.
