Leeds Community Healthcare NHS Trust is currently recruiting Band 5 Occupational Therapists to join our Core Therapy and Active Recovery Neighbourhood Teams. These roles provide an exciting and dynamic introduction to community-based care, supporting patient flow through rapid response, admission prevention, and timely hospital discharge using a Home First approach.

 

Our interventions are varied and impactful, covering areas such as long-term condition management, falls prevention, urgent care to avoid unnecessary hospital admissions, and supporting patients to regain independence at home. You’ll develop a broad and diverse skillset while working holistically with patients, using health coaching and self-management approaches to help them achieve their personal goals.

In this role, you'll have the opportunity to make a real difference in your patients’ lives while developing a broad range of clinical skills. You will work closely with a wider multidisciplinary team, enhancing your knowledge and practice through regular clinical supervision, peer support, and continuing professional development.

 

While you will be fully supported by senior therapists, you will also take responsibility for managing your own caseload—delivering holistic rehabilitation or appropriately delegating care. You’ll be encouraged to work autonomously, develop your clinical reasoning, and strengthen your collaborative working skills, all of which will support your progression within the Trust.

 

This role is part of a 7-day service, with working hours between 8am and 6pm. Weekend and unsocial hours attract enhanced pay. We are committed to supporting a positive work-life balance through our flexible working policy. You will be aligned to either a North or West Leeds hub, with the understanding that this is a city-wide service and occasional cross-cover may be required. We will always consider individual caring responsibilities when planning service needs and rotas.
