We welcome your application to join our community care bed (CCB) team across Leeds. Home first is the ideal outcome for many patients leaving hospital, but a temporary stay in a setting that is a homely environment that can provide 24-hour care is sometimes required. 


The Community care beds do this, enabling early discharge from hospital and reducing hospital admissions and the CCB service delivers complex care whilst empowering patients. as part of a wider multi-disciplinary team. When you join us, we will work with you to develop your own personal developmental plan that supports your learning style. We provide training, clinical supervision and access to a peer support network. We won't just stop there though, we are proud of our career progression pathways that will allow you to develop your clinical, leadership and preceptorship skills. 


Community care beds service work on a shift pattern and cover, 7 days a week. Based in Yeadon, you will be expected to work across all 4 hubs across the CIty as required therfore it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. We do understand that people have busy lives, this post is 18.75 hours per week, offering some flexibility in days and times.

Main duties of the role.


You will provide Therapy interventions between the hours of 8am and 6pm, 7 days a week, (on a rotational basis), your role will be to work with colleagues to assess and provide Physiotherapy interventions and advice as required. You will work as part of a multidisciplinary team with registered nursing, Occupational therapy, technical instructors and you will support the Care home managers and support staff. 


You will work to deliver and facilitate rehabilitation to enable patients to manage their own goals, maximizing opportunities to live a fulfilling life. You will work closely with patients and their families in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of involvement and empowerment. You will aim to improve recovery and increase independence as much as possible by working as part of a multidisciplinary team to achieve the patients goals.
