Vacancy Introduction
The Community Neurological Rehabilitation Service provides support to people, to manage long-term neurological conditions and to live as independently as possible. We aim to empower people to develop, recover or maintain their meaningful activities, independence and quality of life. The community Neurological Rehabilitation Service is available to adults (16+ and no longer at high school) who are registered with a Leeds GP. The team offer goal focused rehabilitation for adults with neurological conditions within a defined period of therapy This may include advice, therapy and treatment for a patient, and advice and support for their family, carers and other professionals. We are a multidisciplinary team made up of a consultant in rehabilitation medicine, dietitians, nurses, occupational therapists, physiotherapists, psychologists and therapy assistants
The post holder will work as part of the Community Neurological Rehabilitation team in the community city wide.
You will work within agreed framework to plan, implement and evaluate clinical and/or therapeutic activities and patient response to activity Working independently but with reference to a supervisor within the relevant clinical area.
You will provide and receive routine or sometimes complex information using persuasion, reassurance, tact and empathy and address barriers to understanding to engage individuals in care and gain valid consent for care.
Prioritise your own workload and understands the boundaries of practice.
Demonstrate clinical leadership in everyday practice through identifying creative and innovative ideas to develop quality service provision and supporting initiatives as an active member of the team.
