Supporting a Home First approach is central to our service. While many patients are able to return directly home following hospital admission, some require a short term stay in a setting that provides 24 hour rehabilitation. Our Recovery Hubs play a vital role in facilitating early hospital discharge and preventing avoidable admissions.
At Wharfedale Recovery Hub, we deliver complex rehabilitation while empowering individuals to regain independence and achieve their goals. You will work as part of a supportive, multidisciplinary team committed to delivering safe, effective and compassionate care. We offer structured training, regular clinical supervision and access to a strong peer support network to support your professional development.
This role operates on a 7 day shift pattern. On occasion, you may be required to work across our other Recovery Hub sites; therefore, applicants must be able to travel independently across the locality and have access to a suitable vehicle for business use.
Main duties of the role
You will deliver physiotherapy interventions between 08:00 and 18:00, seven days a week, on a rotational basis. Working collaboratively with colleagues, you will assess patients and provide appropriate physiotherapy interventions, advice and guidance to support recovery and maximise independence.
You will be an integral member of the multidisciplinary team, working closely with registered nursing staff, occupational therapists and therapy assistant practitioners to ensure coordinated, holistic care. You will contribute to the assessment, planning, implementation and evaluation of individualised rehabilitation programmes, using evidence based and person centred approaches.
A key aspect of the role is supporting patients to identify, work towards and achieve their own goals, promoting self management and enabling people to live as independently and fulfilling a life as possible. You will work in partnership with patients and their families or carers, using inclusive and empowering communication to encourage involvement in decision making and care planning.
You will also contribute to safe and effective discharge planning, provide education and advice to patients, carers and colleagues as appropriate, and support the ongoing development of high quality rehabilitation services within the Recovery Hub.
